The CAB has elected to modify the 2020 ADMC Reaffirmation form due to COVID-19.   Please provide the following information to reaffirm that your DMC continues to comply with the ADMC program standards. You will need your current (or first quarter) Certificate of Insurance to complete this process. The fee for Reaffirmation, which is $125 / year for the 2nd and 3rd year of the accreditation cycle, will be billed separately from this form.
